Important “At-Need” Information and Documents Needed

 

When a Death occurs at Home

If a death happens at home and the deceased IS under HOSPICE CARE, a family member should contact the Hospice representative if they were not present at the time of death and they will let you know the proper procedure to follow. You will need to let them know which Death Care Provider you want to use so they can contact them once they are ready to release the deceased.

If a death happens at home and the deceased IS NOT under HOSPICE CARE or the death was unexpected or accidental, please call 911 to notify the Police/EMS before contacting us. The Police/EMS will be dispatched to the place of death where they will contact the Medical Examiner and/or the deceased’s Primary Care Physician. If the Medical Examiner refuses the case and the Primary Care Physician will sign the death certificate you will need to let them know which Death Care Provider you want to use so they can contact them once they are ready to release the deceased.

If you choose to utilize our services, please have the Hospice nurse or Police Officer call us immediately at (941) 722-3201 ext 1, so that we may bring your loved one into our care.


When a Death occurs at a Facility

If your loved one was in a supervised care facility such as a nursing home, assisted living facility, hospice house, hospital, etc. at the time of death, their staff will notify you and the authorities of their death. If you have let the facility know that we will be caring for the deceased they will contact us when they are ready for us to receive them. Some facilities may allow a verbal release by the legal next of kin and some may require a written release. If you have questions about what may be required please give us a call at (941) 722-3201 ext 1.


Filing the Death Certificate

A Death Certificate must be filed with the Florida State Department of Health within 10 days from the date of death. For us to be able to complete and file the death certificate we will need to collect certain information from the legal next of kin or other authorized agent.

Once received we will enter the information provided into the state system, have the next of kin review the information for accuracy, then send it along to the physician that will be certifying the death for their completion and signature. After the physician has completed the cause of death, the death certificate will be sent to the Medical Examiner for them to review and approve cremation as disposition. Once the Medical Examiner has approved cremation as disposition the record will be filed and registered by the State of Florida.

*The cremation cannot occur until the Medical Examiner has given their approval and we have received the permit from the State of Florida.

Once the record has been filed and registered by the State of Florida we can order any certified copies of the death certificate that are needed from the local Department of Health.

A Cremation Authorization form, signed by the legal next of kin or authorizing agent(s), must be received before we can proceed with cremation. If there is more than one person with the same level of authority, such as children or siblings of the deceased, each person must complete an authorization form.

*The cremation cannot occur without a signed Cremation Authorization form on file with us.

SUBMIT MANUALLY

Use the below link to print the Cremation Authorization form and complete and sign it. If the Cremation Authorization is not signed in the presence of one our staff members, the next of kin or authorizing agent(s) must get their signatures notarized. Once completed & signed, please email or fax us a copy and put the original in the mail to us or bring the original in to our office.

Pay for Services

 

Use the below link to log in to our online portal to pay for the services and/or merchandise that you have already selected. You will need to enter the decedent’s first and last name along with their Account Identifier to access the proper case. Please contact us for the case number.
